What do I need to bring to my visit at La Casa?
At your first visit at La Casa Family Health Center, please bring the following:
- Proof of Identification (A picture identification card, for instance a driver’s license)
- Any medication(s) that you are taken and dietary supplements
- Your medical history-bring any information you may have about your past or present health problems, surgeries, family health history, and immunization records.
- Please inform your Provider/Care Team of any hospitalizations, emergency room or urgent care visits since you were last seen in the clinic.
- Print the Following Forms and bring them with you:
- Forms for Medical
- Forms for Dental
- Forms for Behavioral Health
At Subsequent Visits, please bring the following:

If you have insurance:
- Proof of health insurance: Medicaid or Medicare card, or third party insurance
If you don’t have insurance:
Our admissions staff can determine if you qualify for our sliding fee scale discount program. There are multiple discount levels, based on income and family size. The discount is based from our standard charges and will remain valid for one year after the date of application, unless your income or insurance status changes.
Payment
We accept most types of insurance plans, including Medicare, Medicaid, and most private insurances.
- Payment is expected on the day services are provided.
- Co-pays or the nominal fees are collected during registration.
- You are encouraged to apply for our sliding scale program. If eligible, the program may reduce costs for uninsured patients or be applied to insurance deductibles and co-insurances.
Sliding Fee Scale:
- A sliding fee scale to those who qualify. The fee is based on household income and family size. To qualify, you must provide either a current copy of your tax return, check stub, or a letter from your employer stating your earnings.
- We serve all patients regardless of their ability to pay.
