Our Promotor(a)/Outreach workers play a critical role in helping to break down the barriers to health care faced by low income patients, uninsured, underinsured and other individuals in need that come to La Casa Family Health Center.
Our Promotor(a)/Outreach workers educate and enroll eligible clients into Medicaid, Medicare and other programs. They provide preventative health promotion and family self-sufficiency, they assist in cultural mediation between their communities and health and human services, and they give culturally and linguistically appropriate health education.
The Promotor(a)/Outreach program is available in assisting patients with various needs.
Promotor(a)/Outreach workers are located in Clovis, Portales and Roswell. There are many services that we can link a patient to or can assist in applying for outside services other than health. The following is a list of services available through a Promoter(a)/Outreach worker:
- Presumptive Eligibility/Medicaid On Site Assistance Application (PE/MOSAA)
This program offers on-site assistance with Medicaid for children 0-18 years of age and pregnancy Medicaid for eligible applicants.
- Medicare Savings Plan
This program offers assistance to patients with Medicare, and who are seeking supplemental insurance to help cover cost such as co-payments.
- Disability/Social Security Needs
A promotor(a)/outreach worker can assist patients’ with questions about how to apply for disability or need assistance with forms from the social security office.
- Indigent Programs
Indigent programs are available in Curry and Chaves counties. Qualifying applicants may be eligible for assistance with doctor’s visits, dental visits and prescription cost through La Casa providers.
- One on One Diabetic Patient Case Management
Promotor(a)/Outreach workers will assist patients’ in following up with their diabetic needs as prescribed by their doctors through phone calls and home visits.
- My Chronic Disease Self-Management Workshop
This workshop is available for individuals seeking help with self-managing their chronic diseases. The promtor(a)s are trained in conducting this 6 week course and is available in English and Spanish.
- Child Passenger Safety Information
La Casa Family Health Center has certified and trained Car Seat Technicians in assisting families and children with car seat safety and car seat installation. One on one education and installation is available, or in group sessions.
- Home Visits
For the convenience of our patients, we will offer home visits at the request of a patient or referral through your doctor.
